Download and fill out a PRIMED Consortium Data Sharing Agreement (CDSA), linked below, by following these Instructions for PRIMED CDSA.
Important notes
- If you are unsure whether to sign the CDSA or Amendment letter equivalent, please review the instructions section Can I sign the Amendment letter instead of the CDSA?.
- The PI (Investigator REPRESENTATIVE) should complete Questions 1-4 on the CDSA or Amendment letter prior to sending to your Institution’s Authorized Signing Official. Please review the instructions section Tips for completing CDSA Questions 1-4 to ensure this is completed correctly.
- Please recall that more than one CDSA (separate CDSAs) is needed in certain scenarios such as in the case you are a signing Investigator REPRESENTATIVE for both a PRIMED Study Site (accessing data) as well as study/cohort/biobank/consortium (contributing/sharing data with PRIMED). The CDSA records webpage maintains:
- A list of investigator representatives who have signed the PRIMED CDSA
- A list of studies/cohorts/biobanks/consortia who have signed the PRIMED CDSA
- A list of users who have access to data via a PRIMED CDSA
- A list of AnVIL workspaces containing PRIMED CDSA data
